THE crossing where Lindsey Inger was killed has already come under intense scrutiny since the deaths of Jean Hoggart and her seven-year-old grandson, Michael Dawson, four years ago.
It is a pedestrian-only double crossing which takes users across a tram line and a rail line running side by side.
It is between the bottom of Mill Lane, Bestwood Village, and the roundabout where the A611 Hucknall Lane and Nottingham Road meet and is known as the Bayles and Wylies crossing.
Mrs Hoggart and her grandson died after being hit by a train while crossing the rail side of the crossing on November 22, 2008.
Lindsey died after being hit by a tram on the tram side at about 7pm on Wednesday.
After the 2008 accident, the Rail Accident Investigation Branch made eight recommendations in a report into the tragedy. It called for extra signs and said Network Rail, together with tram operator NET, should reassess the crossing and "establish if the installation of additional protective measures, such as a miniature warning light system, are required".
A miniature warning light system is still not in place at the site.
An NET spokesman said: "Our records show that all the recommendations in the report were taken forward and a review was carried out jointly by Network Rail and NET.
"Signs were erected on the extremities of the crossing.
"The provision of additional protective measures (including miniature warning lights) were evaluated but were not found to be reasonably practicable to implement."
A Network Rail spokesman said all of its recommendations were implemented following the 2008 tragedy.
An inquest into the death of Mrs Hoggart and Michael was told that a tram had passed over the crossing seconds before the accident, so the pair may not have heard the train coming.
A jury at Nottingham Coroner's Court returned a verdict of accidental death.
Mr Hoggart filed a writ to the High Court seeking damages from Network Rail for failing to ensure the crossing was safely lit and that adequate warning signs were in place.
The case was settled out of court.
The amount of damages has not been disclosed.
In September 2009, Janet Sawyer, 52, of West Hallam, Derbyshire, was killed after deliberately stepping out in front of a train on the same crossing.
